Harmony Cares is seeking a Nurse Practitioner to conduct in-home health risk assessments and telehealth visits for patients with complex needs. You will work within the scope of practice as delegated by the Collaborative Physician and coordinate with PCPs to ensure comprehensive care.
Responsibilities include patient evaluations, documentation to NCQA standards, and referrals for case management. Applicants must hold an NP license and board cert, with active BLS and a clean driving record.
