This full-time, non-clinical role focuses on telephonic outreach, care plan support, CMS-compliant documentation, and seamless coordination across the care team.

You will work under RN supervision, handle CHF care planning, and help reduce hospitalizations. Requires MA certification or LVN/LPN, 2+ years in care coordination, familiarity with CMS
