The Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) at Chapters Health System performs fully remote virtual serious illness care management across our continuum, including hospice and palliative care services. You will work with the medical team to assess, plan, and coordinate care for adults and children in home, facility, or inpatient settings.
Requires a Master’s or Doctorate in Nursing, national certification, and current licensure with prescriptive authority; BLS and compact RN
