Humana, Inc. is seeking an RN Care Manager to support Wisconsin Family Care members in Sauk County. This remote role requires traveling up to 40% and coordinating with providers to develop and monitor a comprehensive care plan.
Typical hours are Monday–Friday, 8:00 am–4:30 pm CST, with no nights or weekends. Associate degree in nursing and WI RN license in good standing are required; 1 year of care management experience preferred. Mileage reimbursement provided.
