The Vale Learning Disability Team is a multi-disciplinary team supporting adults (18 years plus). We work closely with other Social Services teams, our Supported Accommodation and Shared Lives Services, external providers, and other agencies.

The Role

Pay Details:
Grade 8-9 (Depending on experience) SCP 26 – 35 £ 36,124– £44,711 per annum

Hours of Work / Working Pattern: 37 hours per week, 8:30 – 5:00pm (4:30pm Friday) with a level of flexibility.

Description:

  • To undertake assessment of citizens in accordance with statutory duties under relevant legislation.
  • To plan and commission services to enable adults with care & support needs and carers with support needs to overcome the barriers to achieving the personal outcomes and monitor and review the care plans through co-production with citizens and carers.
  • To prepare for and participate in supervision and Personal Development Review (#Its About me) sessions with supervisor / line manager, and identify personal development and training needs to maintain Registration with Social Care Wales.
  • To undertake training to support continuing professional development to ensure that the Council is able to retain the provision of its statutory functions, including a willingness for staff to undertake further training to become Approved Mental Health Practitioners, or other more specific, senior roles.
  • To supervise or mentor other staff or students where appropriate and offer input into training programmes and Team Practice Issues meetings where appropriate.
  • To report without delay any safeguarding concerns to the appropriate safeguarding officer.
About You
  • Experience of working with adults and young people who are eligible for support from Social Services.
  • Assessment of Adult citizens and young people.
  • Supporting adults to move on from home / placements into the community.
  • Promoting adults to live as independently as possible and having a fulfilled life.
  • Working in partnership with statutory/voluntary organisations to promote the independence of citizens.
  • Experience of working with a strength-based/person centred approach to supporting citizens to achieve their personal outcomes.
  • Knowledge of relevant legislation eg Social Services and Wellbeing (Wales) Act 2014;
    Mental Health Act;
    Mental Capacity Act etc.
  • Knowledge of Safeguarding issues and procedures in relation to Adults.
  • Knowledge of and commitment to Equal Opportunities policies and procedures.
  • Ability to communicate clearly and effectively, verbally and in writing.
  • Ability to assess need, plan work, set priorities and review arrangements.
  • Ability to analyse and evaluate information.
  • Ability to liaise and negotiate with individuals, professionals and agencies.
  • Ability to work as a member of a team.
  • Ability to work flexibly and under pressure.
  • Ability to operate and maintain IT and casework management systems. A good standard of general education.
  • Commitment to own personal development and further training.
