Fraser is seeking Case Managers to work with individuals in CADI/BI and DD waiver programs. You’ll advocate for clients, connect them to community services, and manage detailed documentation.
The role emphasizes compassion for people with developmental disabilities or related conditions and a strong focus on inclusion and culturally responsive practice. You will handle caseloads of approximately 35–43 clients, with 2–3 meetings per year per client, travel to meet clients 2–3 times weekly, and
