Main duties of the job

In this autonomous and varied role, you will be the key professional for a defined caseload, providing specialist assessment and evidence-based interventions for adults with severe and enduring mental health conditions. A typical week will see you balancing clinical work at our base with community visits, building therapeutic relationships and supporting recovery in a variety of settings.

Your core responsibilities will include co-ordinating care packages, conducting dynamic risk assessments, and regularly evaluating care plans in collaboration with service users, their families, and the wider multi-disciplinary team. You will act as a crucial link between primary and secondary care services, advocating for your patients and ensuring their physical and social well-being is holistically addressed.

Job responsibilities

We are seeking an experienced and highly motivated Mental Health Practitioner to join our busy Community Mental Health Team (CMHT) on a locum basis. You will be responsible for managing a defined caseload of service users with a range of severe and enduring mental health problems, providing specialist assessment, planning, implementation, and evaluation of care.

Working as an autonomous practitioner within a supportive multidisciplinary team, you will deliver high-quality, evidence-based interventions in a community setting. The role is split between our team base and community visits, requiring a car driver.

2. Main Duties and Responsibilities

Clinical Practice:

To manage a designated caseload, acting as a Care Co-ordinator under the Care Programme Approach (CPA) for service users with complex needs.

Conduct comprehensive, person-centred assessments of service users, including risk assessment and management.

Develop, implement, and regularly review individualized care plans in collaboration with service users, their families/carers, and other professionals.

Provide a range of evidence-based therapeutic interventions to support recovery and promote mental well-being.

Monitor and evaluate treatment outcomes, making adjustments to care plans as necessary.

Professional Responsibilities:

Work effectively as part of a multidisciplinary team (MDT), contributing to team decisions and MDT meetings.

Maintain accurate, timely, and concise clinical records in line with professional and Trust standards.

Act as an advocate for service users, ensuring their views are heard and their rights respected.

Communicate complex and sensitive information effectively to service users, families, and other agencies.

Health and Safety:

To be responsible for the safe and effective use of any equipment or resources used in the course of their duties.

To adhere to all Trust policies and procedures, including those relating to safeguarding, health and safety, and infection control.

Person Specification Qualifications
  • Professional

    Qualification:

    Registered Mental Health Nurse (RMN) with valid NMC registration, or an equivalent professional qualification (e.g., Social Worker, Occupational Therapist) with relevant registration.
