Mersey and West Lancashire NHS Foundation Trust is seeking a highly motivated and experienced Band 8a Clinical Pharmacist to join our dedicated team at Southport and Formby District General Hospital. This role offers a fantastic opportunity for an enthusiastic pharmacist to play a key role in delivering high‑quality clinical pharmacy services and optimising medication management for inpatients.
Responsibilities- Taking drug histories on admission and completing medicines reconciliation for inpatients.
- Screening newly admitted patients for medicines optimisation issues and developing individualised care plans.
- Conducting medication reviews to ensure safe, evidence‑based, and cost‑effective prescribing.
- Monitoring and adjusting patient medication based on therapeutic drug monitoring results and response to treatment.
- Counselling patients on their medication, ensuring they understand their drug therapy and any potential side effects.
- Providing professional and legal clinical supervision in the dispensary, ensuring best practices in dispensing and medicines management.
Essential Criteria
- Master's degree in pharmacy (MPharm) or equivalent qualification.
- Registration with the General Pharmaceutical Council (GPhC).
- Evidence of ongoing Continuous Professional Development (CPD)/Revalidation in line with professional requirements.
- Previous relevant hospital experience in a clinical setting.
- Strong foundation in Clinical Pharmacy and Medicines Optimisation.
- Excellent verbal and written communication skills to liaise effectively with multidisciplinary teams.
- Strong time‑management, decision‑making, and organisational skills.
- Computer literacy and experience using clinical systems and electronic prescribing tools.
- Postgraduate qualification in Clinical Pharmacy or independent prescribing qualification.
- Experience in a specialist clinical area such as acute medicine, critical care, or cardiology.
- Previous experience supervising and mentoring junior pharmacists or pharmacy technicians.
- Familiarity with NHS policies and procedures in medicines management.
