What You Will Do

Carry out needs assessments for adults in the community to develop effective, personalised care plans.

Work collaboratively with colleagues, external partners, and agencies to ensure holistic, high-quality support.

Conduct timely reviews of care and support plans to ensure they remain appropriate and effective.

Provide advocacy, guidance, and support to service users, promoting independence and wellbeing.

Maintain clear, accurate, and compliant case records in line with legal and organisational standards.

Engage in ongoing professional development, keeping up to date with legislation, policy changes, and best practice.
