LOCATION AND ABOUT NHH
ROLE OVERVIEW
The part-time Occupational Therapist (OT) in the Stroke Recovery Clinic provides OT assessment, intervention, education and discharge planning of referred stroke clients in the areas of self-care, productivity and leisure. The OT practices in accordance with the Professional Standards of the College of Occupational Therapists of Ontario (COTO). This currently vacant role operates shifts, Tuesday and Thursday, weekly.
Primary Responsibilities
Receive, prioritize, and schedule appropriate stroke rehab referralsReview past assessments and historiesSelect assessment tools to determine the individual's functional abilitiesPerform assessment, including consulting with family/caregiverEvaluate, document and communicate the results to client/family/team membersDevelop and document an intervention plan based on analysis of assessment and expected outcomes of stroke severity in consultation with the client and or familyDevelop intervention plans with goals that are clear, measurable, functional and appropriate to individual's needs, personal goals and expected outcomesDetermine treatment techniques and application of assistive devices in consultation with other health care professionals including ADP authorization as requiredConsult with other community health care providers by written report or phoneReassess and document the individual's level of functioning and changes in occupational performance and modify the treatment accordinglyEducate caregivers and family members in activities that support the intervention planPractice personal and patient related safety precautions in accordance with COTO and hospital guidelinesCoordinate with the interdisciplinary team and other community resources including family membersUse appropriate outcome measures to determine when the client has achieved goals or maximum benefit from OT services or is no longer appropriate for OT intervention due to changes in health statusMaintain patient records as outlined by hospital policy and procedures and consistent with COTO guidelinesContributes to the program by performing related tasks as requiredModels NHH Philosophy and ValuesMay be required to support inpatient allied health team related to HHR and hospital demandsQUALIFICATIONS/REQUIREMENTS
Completion of a university degree in Occupational Therapy from a recognized University or equivalent.Licensed to practice with the College of Occupational Therapists of Ontario.Excellent communication skillsExcellent interdisciplinary team skillsGood decision making and deductive reasoning skillsGood interpersonal skills and ability to work effectively with patients, families, other professionals, and interdisciplinary team membersGood documentation skillsProfessionalism, independence and dedication to client centred practiseGood organizational and time management skillsComputer and keyboarding skillsNew graduates will be consideredPHYSICAL EFFORT…
