LOCATION AND ABOUT NHH

ROLE OVERVIEW

The part-time Occupational Therapist (OT) in the Stroke Recovery Clinic provides OT assessment, intervention, education and discharge planning of referred stroke clients in the areas of self-care, productivity and leisure. The OT practices in accordance with the Professional Standards of the College of Occupational Therapists of Ontario (COTO). This currently vacant role operates  shifts, Tuesday and Thursday, weekly.

Primary Responsibilities

  • Receive, prioritize, and schedule appropriate stroke rehab referrals
  • Review past assessments and histories
  • Select assessment tools to determine the individual's functional abilities
  • Perform assessment, including consulting with family/caregiver
  • Evaluate, document and communicate the results to client/family/team members
  • Develop and document an intervention plan based on analysis of assessment and expected outcomes of stroke severity in consultation with the client and or family
  • Develop intervention plans with goals that are clear, measurable, functional and appropriate to individual's needs, personal goals and expected outcomes
  • Determine treatment techniques and application of assistive devices in consultation with other health care professionals including ADP authorization as required
  • Consult with other community health care providers by written report or phone
  • Reassess and document the individual's level of functioning and changes in occupational performance and modify the treatment accordingly
  • Educate caregivers and family members in activities that support the intervention plan
  • Practice personal and patient related safety precautions in accordance with COTO and hospital guidelines
  • Coordinate with the interdisciplinary team and other community resources including family members
  • Use appropriate outcome measures to determine when the client has achieved goals or maximum benefit from OT services or is no longer appropriate for OT intervention due to changes in health status
  • Maintain patient records as outlined by hospital policy and procedures and consistent with COTO guidelines
  • Contributes to the program by performing related tasks as required

  • May be required to support inpatient allied health team related to HHR and hospital demands
  • QUALIFICATIONS/REQUIREMENTS

  • Completion of a university degree in Occupational Therapy from a recognized University or equivalent.
  • Licensed to practice with the College of Occupational Therapists of Ontario.
  • Excellent communication skills
  • Excellent interdisciplinary team skills
  • Good decision making and deductive reasoning skills
  • Good interpersonal skills and ability to work effectively with patients, families, other professionals, and interdisciplinary team members
  • Good documentation skills
  • Professionalism, independence and dedication to client centred practise
  • Good organizational and time management skills
  • Computer and keyboarding skills
