We are seeking a SWAN Nurse to travel throughout our Transitional Care Units in GTA Durham and NSM.

Job Qualification

Qualifications you currently have:

Completed CNA-accredited Skin Wellness Associate Nurse course

Registered Practical Nurse who graduated from an accredited School of Nursing

Registered as a member in good standing with the College of Nurses of Ontario

Must have a valid driver’s license and reliable vehicle

Two years of previous experience in the development of wound care management programs in a hospital or community setting

Previous visiting nursing experience will be considered an asset

Updated immunization records, clear criminal background check, and vulnerable sector check upon hire

Current CPR certification required
