Physiotherapist – Champlain Rehab Solutions

Estimated annual earning potential: $80,000 – $130,000 (actual earnings may vary based on availability and service volumes).

• Assess clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines.
• Implement client interventions based on assessment information, client input and the integrated care plan.
• Ensure services are best practice, evidence‑based and evaluated through valid and reliable outcome measures.
• Attend client‑related meetings to report on assessment findings and discuss plans for intervention, progress reports and discharge as required.
• Coordinate delivery of care through ongoing communication with all members of the client’s team.
• Document all assessment findings, treatment plans, progress and discharge summaries as required by the profession, referral source and the organization.
• Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health and other agencies.
• Adhere to the code of ethics of the profession and maintain standards of practice for the Ontario College of Physiotherapists.

• Baccalaureate or Master’s degree in Physiotherapy from a recognized school.
• Registered, in good standing, with the College of Physiotherapists of Ontario.
• Minimum of 2 years clinical experience, preferably in the community.
• Relevant specialized courses to support clinical expertise.
• Clinical experience with a variety of physiotherapy interventions.
• Knowledge of community care requirements and available resources.
• Excellent consultation and teaching skills, utilizing an evidence base in clinical practice.
• Familiarity with routine use of valid and reliable outcome measures.
