Job Summary
We are hiring dynamic, passionate Physiotherapists (PTs) who want to be part of the transformational change in home and community care by providing care in a truly client-centered approach. You will be part of a diverse care team that will provide services to a roster of clients in the community. You will be part of a collaborative team that will conduct joint care planning with a focus on client-centered goals, addressing the client’s physical, cognitive, social, emotional, and spiritual domains, and creating a holistic care plan. Each client is assigned a primary care clinician, a member of the interdisciplinary team who will lead care planning for that client and facilitate the interdisciplinary team to reach the client’s care goals.
As part of the SE Transitions team, you will be a part of the team that will be accountable for the care and services you provide and the outcomes your clients achieve.

Job Responsibilities- Conducting assessments
- Formulating goals and recommendations
- Planning and implementing client-centered, goal-directed treatment programs
- Graduate of an accredited Physiotherapy program and member in good standing with the College of Physiotherapists of Ontario
- Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred
- Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with rehab assistants as well as with a high-performance interdisciplinary rehabilitation team
- Sound knowledge of community resources is an asset
- Effective interpersonal, organizational and communication skills
- Positive work and attendance record is essential
- Ability to adapt to new computer applications
- Hybrid work approach: able to work remotely and with clients in their acute/rehab or community-based rehabilitation setting
- Vehicle and valid driver’s license and ability to travel

CompensationHiring pay range: $38.00 - $45.00 Paid Per Visit, based on experience.
