Ontario Health atHome is seeking a bilingual Nurse Practitioner to join our Regional Palliative Consultation Team on a temporary part‑time basis (0.2 FTE). The role focuses on delivering patient‑centered, holistic palliative care to medically complex patients in the home setting.
Responsibilities- Provide comprehensive primary health care services, including medication reconciliation, prescribing, ordering laboratory work, chart reviews, and physical examinations for at‑risk adults and seniors.
- Liaise with physicians, community partners, and patient families to coordinate care and address patient needs and goals.
- Deliver health promotion and education tailored to each patient’s needs within the Specialty Programs.
- Conduct focused assessments, review intake notes, coordinate with Care Coordinators, and provide recommendations to the Care Coordination team.
- Consult and support Ontario Health atHome Champlain Care Coordinators and key stakeholder partners.
- Advise on best practices, propose improvements, and collaborate on the development of policies, procedures, and joint initiatives with community partners.
- Participate in regular professional development, academic rounds, and educational events.
- Membership in good standing with the College of Nurses of Ontario.
- Registered Nurse in the Extended Class (RN EC designation).
- Fluent in English and French.
- Working knowledge of community resources and the roles of health‑care professionals.
- Experience with medication reconciliation and care transitions.
- Strong communication and team collaboration skills.
- Ability to provide safe, effective care in a home‑based, palliative context.
- Specialized education in palliative pain and symptom management.
- Experience delivering palliative or end‑of‑life care.
- Knowledge of Medical Assistance in Dying legislation.
- Experience with complex chronic disease management and psychogeriatric care.
- Advanced assessment skills and sound clinical therapeutic knowledge.
- Demonstrated clinical leadership and collaborative practice with all care providers.
One day per week, Monday to Friday – 8:30 am to 4:30 pm (0.2 FTE). First assignment starts March 27 2026.
Compensation and Benefits- Salary: $44,480 – $47,517 per year.
