Job SummaryThe post-holder will continue to support the community nursing team but with a wider knowledge and skill set. The successful applicant will demonstrate a range of clinical skills and competencies along with excellent communication skills, providing advice and information to clients and carers with the aim of maintaining them in their own homes, enabling maximum independence and optimum quality of life.
Responsibilities- Primarily support the community nurse team in delivering nursing care to clients with long‑term conditions and complex health and social care needs across the community nursing and Intermediate Care Service.
- Support the team in the facilitation of early discharge from hospital where appropriate.
- Provide advice and information to clients and carers to help maintain them in their own homes.
- Foundation Degree in Healthcare Practice (Nursing Associate)
- Trust induction training
- Experience in a Community Setting
- Understand and demonstrate evidence-based practice.
- Understanding of when to seek advice and refer to a registered care professional.
- Understanding of the scope of the role of the Nursing Associate in the context of the team and the organisation, and how the role may contribute to service development.
- Intermediate IT skills.
- Understand and demonstrate the importance of the promotion of health and wellbeing.
- Ability to work independently with a delegated caseload, use initiative whilst understanding limits of scope.
- Ability to work effectively as a team player under appropriate supervision, and as part of a multidisciplinary team.
- Ability to support the development of less experienced staff.
