MJHS Home Care is seeking a Home Care Planner to evaluate and establish plans of care for patients referred to home health services. The role supports admission from hospital to home care and collaborates with physicians, social workers, and discharge planners to meet patient needs.
You will meet monthly admission targets, deliver outstanding customer service, and actively cultivate referrals while supporting MJHS services such as Home First, Palliative Care, and Hospice.
