In partnership with families and communities, we provide outpatient and community-based services with the aim of enriching the independence and wellness of those we serve ~ Building Abilities for Life.
We assess and treat clients from infancy to their late teens, offering a wide variety of services including physiotherapy, occupational therapy speech and language therapy, social work and therapeutic recreation. We provide treatment in schools, in the community and at our locations in Peterborough, Lindsay, Cobourg, Campbellford and Minden.
FCCC is proud of the partnerships we have developed within the community. These partnerships have assisted families in our communities to access additional services that are needed to assist with their child’s development.
About the Job:We are accepting applications for a contract, part-time Central Intake Coordinator (working 21 hours/week) in our Peterborough location. This is a 12-month contract from August 2025 to August 2026.
In this position, you will be responsible for determining client eligibility for Centre services. You will manage Centre referrals and coordinate supported connections for clients and families, the community and Centre partners. You will manage operation of the Smart Start Hub in support of clients/families, the community and our partners to access developmental services for children and youth in our Centre regions.
Your key responsibilities will include:
- Coordinate intake of Centre external and internal referrals.
- Complete intake assessment (About My Child, About My Baby, About Me assessments) and coordinates follow up with client/family/provider/partner as a Smart Start Hub agency.
- Determine eligibility considering FCCC catchment area(s), program/service type, mandate and urgency.
- Liaise with community partners to ensure supported connections for appropriate services and resources for clients/families.
- Comply with Centre’s policies and procedures as amended from time to time.
- Maintain strict confidentiality regarding all information pertinent to the children and families served by the Centre.
- Assume other duties as assigned in accordance with experience, training, and qualifications to which legislated requirements may be applicable.
What You Bring:
- College Diploma in one of the following:
Communicative Disorders Assistant, Physiotherapy/ Occupational Therapy Assistant or Social Service Worker. - 3-5 years working in a community/health care environment with direct contact/interaction with clients.
- Proficient knowledge of medical terminology.
- Ability to work independently as well as part of a team.
- Demonstrated ability in communication, organizational skills and problem solving.
- Be committed to working within a family-centred approach.
- Demonstrated ability to work collaboratively with FCCC staff and with other professionals in the community.
- Updated police check (within last 12 months).
- Proof of updated immunizations including COVID-19 (two doses).
- Competitive hourly rate. $29.71-$33.55, commensurate with experience.
