We are seeking an experienced Practice Nurse tojoin our dedicated team. This is an exciting opportunity for a motivated and skilled nurse to contribute to the delivery of safe, effective and high-quality care within a primary care setting. The successful candidate will work withinthe NMC Code of Conduct, supporting service development and promoting integrated care pathways for patients.
Please note this is a part time role offering 20 hours a week and the salary advertised is the pro rata'd amount.
Main duties of the jobThe Practice Nurse will:
- Deliver primary health care interventions and treatments in line with clinical protocols.
- Provide health education and promotion to patients and carers.
- Undertake clinical duties such as wound care, immunisations, cervical screening, phlebotomy, ECGs, and chronic disease monitoring (e.g., diabetes, asthma, COPD).
- Work collaboratively with GPs, nurses, and other healthcare professionals to ensure seamless patient care.
- Maintain accurate patient records and contribute to audits and quality improvement initiatives.
- Support infection prevention and control, safeguarding, and health & safety standards.
- Participate in team meetings, training, and professional development activities.

About the Practice/Department/Team
Oundle Medical Practice is part of Lakeside Healthcare, and a member practice within Rockingham Forest Primary Care Network(PCN).Based in the quintessential market town on the River Nene, serving a diverse patient population, the surgery has a strong commitment to delivering high-quality, patient-centred care.
Job responsibilities- Responsible for providing and delivering primary health care interventions and care effectively and efficiently
- Working collaboratively with the multi-disciplinary general practice team and other healthcare professionals to promote integrated pathways of care to meet patient needs.
- Effective communication with patients, carers and all members of the MDT
- Effective prioritisation and completion of daily work in line with clinical / patient need and practice priorities
- Registered Nurse
- Degree in healthcare/nursing
- Mentorship course or equivalent
- Primary Care Nursing Qualification
- Accredited Diploma level training in chronic disease management
- Clinical supervision training and experience
- Recent experience as a Registered Nurse in a primary care setting
- Evidence of continued professional development
- Experience of managing/dealing with the public/patients
- Experience of audit and/or research
Experience of using clinical systems and Microsoft office - Management of long-term conditions
- Participation in quality initiatives such as clinical benchmarking
- Experience of Systm One
