Salary Range: $60,450.00 – $80,000.00 per year.

- Complete a comprehensive initial and interim care assessment within expected timelines as per the contractual agreement and service program, utilising tools, forms, and processes aligned with CBI Home Health policy and procedure, documenting all required information.
- Ensure client and care provider safety by using risk assessment tools provided.
- Respond to client complaints, striving to resolve when possible and escalating as required internally (to management) and externally (to funders).
- Identify opportunities to improve service and/or delivery, engaging appropriate team members to drive effective and efficient change.
- Ensure the client/client family experience is satisfactory and that they receive all required information relevant to their care.
- Collaborate with the interdisciplinary team and internal/external stakeholders on service delivery delegations to ensure care needs and quality standards are met.
- Lead by example by promoting service delivery excellence through effective onboarding, mentoring/coaching, and managing performance of field staff, including conducting and documenting 30‑60‑90‑day check‑ins and annual performance reviews.
- Lead regular team meetings and/or individual meetings, and send regular communication to field staff regarding pertinent updates.
- Ensure all care provider team members fulfill accountabilities per their job description and meet expectations through regular touch‑points and observation at drop‑in visits.
- Ensure all reporting (internal and external) meets or exceeds all standards and expectations of CBI Home Health, stakeholders, and the associated regulatory body (if applicable).
- Ensure appropriate documentation is completed accurately and on time, including client charting as per regulatory body and funder agreements.
- Complete review and follow‑up of practice management system reports to monitor and assess client documentation, GPS tracking, and visit compliance.
- Possess a current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse or Registered Practical Nurse.
- Have at least 2 years of supervisory experience and/or experience in home health service delivery.
- Computer skills – proficiency with Microsoft Office Suite, MS Teams, and scheduling systems.
- Ability to establish and build rapport with clients and care staff.
- Valid Ontario driver’s licence with reliable vehicle and ability to travel within the community.
- Ability to provide best‑possible solutions to problems aligned with policies and best practices.
- Self‑motivated with ability to work both independently and as an effective team member.
