Resident Care Coordinator; LPN​/RN Lead & Elevate Care

Spring Oak Senior Living is seeking an experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to become the Resident Care Coordinator for our Petersburg campus, overseeing care across both assisted living and memory care buildings.

You will train and support CNAs and care staff, coordinate assessments and care plans, manage medications, and ensure compliance with Virginia regulations and HIPAA. On-call rotation is required.
