Inpatient Rehabilitation LPN​/LVN

Role: LPN/LVN providing compassionate bedside nursing care to adult rehabilitation patients with stroke, brain and spinal cord injuries, complex orthopedic/neurological conditions, and other serious illnesses.

Key Responsibilities

• Administer medications and prescribed treatments.
• Monitor and record vital signs and patient condition.
• Document observations and care activities accurately.
• Support rehabilitation therapy goals in coordination with RNs and an interdisciplinary team.
• Promote patient safety, education, and independence.
• Contribute to care planning and communicate with patients’ families.
• Maintain a positive, team-focused environment grounded in clinical excellence and respect.

Qualifications & Skills

• Active LPN/LVN license.
• Strong clinical assessment and monitoring skills.
• Ability to follow treatment plans and administer medications safely.
• Proficient documentation and communication skills.
• Compassionate bedside approach and commitment to patient-centered education.
