The Dementia Advisor Service are looking for an enthusiastic full or part-time Dementia Advisor to support people across the Plymouth area who have a diagnosis of Dementia or Mild Cognitive Impairment.

The role will be based at Mount Gould Hospital and will provide group or individual support across the Plymouth area to help people with a Dementia or Mild Cognitive Impairment diagnosis, to develop a plan to support them and their families and/or carers in meeting their needs, enabling people to maintain as much independence as possible, in a place of their choosing, in a manner of their choosing and to live well within their diagnosis.

The post holder will do this by delivering educational programmes in the community, offering support and guidance and working with other agencies statutory or voluntary in an enabling manner.

Main duties of the job

Provide support, information and guidance to people with a dementia or mild cognitive impairment diagnosis and their family and/or carers, helping them to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. To assist people to identify their needs and access to services.

This will be achieved through:

  • an educational programme, delivered in a group or individual setting.
  • supporting patient and/or carers in the development of a support plan that is enabling and empowering and allows them to make choices that are meaningful for them.
  • 12 monthly reviews of 'My Dementia Plan' with patient and/or carers.
  • signposting to other agencies statutory or voluntary.
  • providing support to people approaching the service for guidance and signposting around obtaining support or diagnosis
  • promoting principles around living well with Dementia or Mild Cognitive Impairment and in a person-centred manner
  • maintaining accurate and timely electronic and written records in relation to contact with users of the service in accordance with organisational policy and in best practice around data protection and governance principles
  • participating in service development and planning.
  • working as part of a team.
  • This role may not be eligible for sponsorship under the Skilled Worker route.
  • All staff are expected to be able and willing to work across a 7 day service.
