Empath Health is seeking an RN Case Manager to begin at admission and coordinate comprehensive, patient- and family-centered hospice care across North Port and Englewood. You will develop and maintain the care plan, liaise with clinicians, and guide families through the dying process with empathy and clinical expertise.
The role requires active Florida RN licensure, an approved nursing program, and at least one year of acute-care experience; hospice experience is preferred.
