We are currently recruiting a Nurse Practitioner (NP) to provide comprehensive, integrated, and trauma-informed care at our Urgent and Primary Care Centre (UPCC) located in Port Moody, B.C. Port Moody is located within the Tri-Cities, with extensive access to parks, nature, transit, vibrant local culture, and growing, diverse communities.

UPCCs are dedicated to delivering timely, person-centred, culturally safe, trauma-informed longitudinal and urgent primary care. Our model of care offers holistic team-based care by supporting physicians, nurse practitioners, nurses, social workers, clinical counsellors, allied health professionals and administrative support. Together this team will extend continuous care that will be intentional and coordinated to all members of the community, including the vulnerable and medically complex.

The Tri-Cities is an informal grouping of three lively suburban communities of Coquitlam, Port Coquitlam, and Port Moody and two semi-rural residential areas of Belcarra and Anmore that have grown together and are each unique from the other. Be part of our clinical enhancement program with regular in-house skills development such as 12 lead ECG interpretation, Point of Care ultrasound (POCUS), suturing and x-ray interpretation in addition to receiving centralized NP orientation. You will be welcomed into a New Graduate NP Community of Practice team helping to support your successful transition into independent practice.

The Nurse Practitioner is responsible and accountable for the comprehensive assessment of patients/clients/residents including diagnosing diseases, disorders, and conditions. The Nurse Practitioner initiates treatment including health care management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits, and conditions, and employer policies and procedures. The Nurse Practitioner provides professional guidance to other health professionals and practices autonomously and interdependently within the context of an interdisciplinary health care team, making referrals to specialist physicians and others as appropriate.

The position collaborates with patients/clients/residents and other health professionals to identify and assess trends and patterns that have implications for patients/clients/residents, families and communities; develops and implements population and evidence based strategies to improve health and participates in policy-making activities that influence health services and practices. The position participates in peer review and self review to evaluate the outcome of services at the patient/client/resident, community and population level.
