Join the Home Health interdisciplinary team as a Clinical Social Worker to provide assessment and treatment services for clients. You will liaise with community agencies, support caregivers, and participate in educational services, program development, and policy recommendations. The role supports the Home Health team in delivering client-centered care within the assigned area of responsibility.

- Provide therapeutic counselling services to clients and families experiencing complex medical diagnoses.
- Conduct social work and psychosocial assessments of clients and families using professional standards and clinical practices.
- Follow up and monitor ongoing psychosocial situations; evaluate client response to interventions.
- Collaborate with the interdisciplinary team to develop comprehensive care plans and share client psychosocial information.
- Act as a resource for community resources and facilitate referrals to support client families.
- Participate in quality improvement activities and provide input into program goals, objectives, policies, and procedures.
- Promote client autonomy and ensure participation in planning and decision making.
- Assist in developing educational programs for home health care staff and community agencies on psychosocial health issues.
- Prepare statistics for workload measurement and maintain written clinical records.
- Perform other related duties as assigned.
- Master's Degree in Social Work from an approved school of Social Work.
- Two (2) years of recent related experience in a complex health care environment (e.g., community or home health setting).
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's License and access to a personal vehicle for business‑related purposes.
- Applicants with a Bachelor of Social Work (BSW) degree may be considered if accompanied by an equivalent combination of education, training, and experience. Please outline how your knowledge, education, skills, and experience meet Master of Social Work (MSW) equivalency in your application.
- Knowledge and understanding of counseling skills and clinical practice models for individuals, families, and groups.
- Ability to conduct social and psychosocial assessments.

- Understanding of scope of practice of other health care disciplines.
- Referrals and community resource knowledge.
