We are looking for a supportive, caring and patient person to work independently with two men with learning disabilities in Prestatyn.

Support the following individuals: one who enjoys cycling and meeting friends, and another who enjoys swimming, walks, horse riding and fish and chips. You will help them maintain independence and self‑sufficiency, encourage participation in community activities and assist with daily living tasks.

• Provide person‑centred support to two men living in their own home.
• Assist with personal care, medication management, meal preparation and household maintenance.
• Encourage and facilitate participation in leisure and social activities.
• Maintain accurate records of care and communicate progress to service manager.

• Full induction, training and ongoing support will be provided – learning disability experience is advantageous but not required.
• Valid driver’s licence and experience driving in a support setting.
• Familiarity with health and safety standards in a community care environment.
• Desire to work collaboratively with a supportive and inclusive team.
