Geriatric Medicine Job Prince George area,BC Canada,Doctor/Physician

The University Hospital is seeking a Geriatrician to provide inpatient and outpatient geriatric services at various sites. The successful applicant will become a member of the Department of Internal Medicine but will not be required to participate in the call rota.

Geriatric inpatient and outpatient services include, but are not limited to:

Assessment, treatment, and follow-up in outpatient clinics or in the hospital;

Medication review and education;

Referrals from family physicians or attending specialists for elderly patients living in the community or in long-term care facilities;

Referrals from physicians for hospitalized elderly patients with medical, functional, or psychosocial issues interfering with discharge or community functioning;

Complex competency assessments in collaboration with psychiatry;

Discharge planning;

Geriatric medical consultation and diagnosis;

Development of treatment and management strategies;

Consultation with community-based multidisciplinary teams for seniors, including care planning;

Consultation with the patient's family physician and family;

Provision of consultation and clinical program enhancement activities, including planning and evaluation of geriatric services and resources;

Collaboration with staff and professionals to develop and deliver effective geriatric services;

Participation in evaluating service quality and efficiency through audits, reviews, and incident reports.

Performance Deliverables

New outpatient clinic assessments;

Follow-up assessments (acute and outpatient);

Consultations (acute and outpatient);

Development of outreach and telehealth consultation services to support patients in underserved areas, in collaboration with Northern Health and the Geriatrician.
