Clinics – onsite / Case Management - remote
Note: All applicants must have a current and valid RN or LPN license in British Columbia and have the ability to travel with the mobile clinic to Northern communities of British Columbia 1-2 weeks per month.
As an integral part of a Mobile Diabetes Clinic, the Diabetes Nurse implements and delivers a range of diabetes services to patients from First Nations communities in Northern BC. These services include testing and diagnoses as well as health promotion and prevention objectives aimed at increasing First Nations access to diabetes care/education and improving overall health outcomes.
Duties & Responsibilities- Provide diabetes testing and screening services and provide diabetes education individually and/or in group settings, as well as conduct home and follow-up visits to support diabetes care and overarching health objectives.
- Advocate for First Nations affected by diabetes and related health conditions.
- Plan and implement diabetes care goals across the lifespan (high risk individuals, Pre-Diabetes, Gestational Diabetes, Type 2 Diabetes, etc.).
- Evaluate client results and provide pertinent teaching, goals, and recommendations to clients, families, and caregivers.
- Promote awareness of diabetes and related conditions through conference presentations, information booths, and major community events.
- Current and valid registration as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with the British Columbia College of Nurses and Midwives (BCCNM).
- Certified Diabetes Nurse Educator is strongly preferred.
- Experience working within the scope of practice and guided by the 2018 Diabetes Canada Clinical Practice Guidelines (CPG) for the Prevention and Management of Diabetes in Canada.
- Knowledge of diabetes testing, diagnosis, and management techniques.
- Knowledge of First Nation adult learning styles and innovative teaching methods applied to individual and group settings as well as their culture and traditions is strongly preferred.
