Position: Licensed Practical Nurse (LPN)

Position overview

The Licensed Practical Nurse (LPN) provides comprehensive nursing services within the Wolf Creek Primary Care Network (WCPCN). Working to full scope of practice, the LPN supports patients and physicians through the provision of general primary care nursing services. The LPN collaborates closely with physicians, PCN team members, and community partners to improve patient outcomes and support accessible, high-quality primary care.

The role requires strong clinical judgement and critical thinking, excellent communication, and the ability to work both independently and as part of a multidisciplinary team. Duties may vary according to clinic needs, population served, and program priorities.

Key Responsibilities
  • Work collaboratively with physicians, patients, and families in delivering primary care services.
  • Build therapeutic relationships and provide patient-centered health education.
  • Conduct assessments, implement nursing management strategies, and provide follow-up in alignment with protocols and scope of practice.
  • Exercise appropriate autonomy in clinical decision-making while maintaining collaboration with medical practitioners.
  • Track, monitor, and report patient encounters and program indicators.
  • Participate in quality improvement activities and PCN initiatives.
  • Other responsibilities as requested.
General Clinical

Provide health services aligned with LPN scope, including:

  • Assistance with wound care, injections, and other clinical procedures relevant to all areas of primary care, including prenatal care.
  • Support health promotion activities and referrals to PCN or AHS programs.
  • Participate in case finding, panel-based care activities, and population health screening initiatives.
  • Monitor patient progress and response to treatment, and recommend adjustments to care plans and appropriate investigations to physicians, as necessary.
  • Coordinate with public health and community services to enhance transitions of care for the patient population.
  • Work both independently and in collaboration with other healthcare professionals to ensure high-quality care is provided to the patient population.
Administrative Responsibilities
  • Document relevant clinical information in a timely manner in the EMR and ensure the maintenance of accurate patient records.
  • Assist in developing and implementing health promotion programs relevant to the needs of the practice population.
  • Assist in developing, implementing, and monitoring the program evaluation framework and data collection for the LPN role.
  • Assist in developing and implementing patient self-management programs.
  • Act as a resource for other Primary Care Network team members.
  • Provide training to other team members and assist in their professional development.
Qualifications and Experience
  • Diploma in Practical Nursing from an accredited program.
  • Active registration and good standing with the College of Licensed Practical Nurses of Alberta (CLPNA).
  • Current BLS certification.
  • Strong assessment and clinical decision-making skills within LPN scope.
  • Preference given to those with at least 5 years nursing experience.
  • Experience in some or all the following areas is desirable:
    Primary Care/Community Nursing and Women's Health.
  • Confidence, proficiency and experience in using EMRs and Microsoft Office (specifically Outlook and Word).

  • The ability to establish and nurture good rapport with patients, service providers and professionals in the surrounding communities, and the ability to develop and maintain collaborative and cooperative relationships in a multi-disciplinary team, and with community partners/stakeholders.
  • The ability to recognize gaps in the current level of services provided and report opportunities for continuous process improvement to the PCN management team.
  • A desire to either live in the rural community being served or at least a strong vested interest in the health of the community in order to become familiar with the needs, resources and particular issues unique to each community.
  • Excellent written and verbal communication skills.
  • Excellent organizational, prioritization and time-management skills.
  • A high-level of compassion, respect, integrity, ethics and confidentiality.
  • The ability to work independently; in a highly complex, unstructured, and dynamic environment.
  • Client focused orientation and commitment to providing quality services and programs.
  • Ability to organize time, multi-task and set priorities to meet various deadlines and maintain a flexible schedule and time management.
  • Must provide proof of certification annually.
