We are looking to recruit an experienced Nurse Practitioner to support the clinical team in our busy GP Surgery in Redcar.
You will demonstrate safe clinical decision making andexpert care, working collaboratively with the general practice team to meet theneeds of patients.
Main duties of the jobThe post holder will be competent in the delivery of primary care to our practice population, having direct clinical involvement and extensive clinical knowledge.
They will assess, diagnose, treat, refer or signpost patients who attend with undifferentiated or undiagnosed conditions.
They will work as an independent and autonomous Care Practitioner providing a first point of access in primary health care.
The post holder will undertake, home visits, including care homes primarily for acute conditions, as assessed by the duty doctor and within boundaries of professional qualification.
They will also be able to provide clinical supervision and support to the nursing team, and advise and support while liaising with other practice staff.

Job Summary

They will work atan independent and autonomous Care Practitioner level providing a first pointof access in primary health care
To undertake home visits, including
Nursing/Residential Homes primarily for acute conditions, as assessed by yourduty doctor, and within the boundaries of professional qualifications.
The post holder will use advanced clinical skills to provide education to patients, promoting self-care and empower themto make informed choices about their treatment.
They will be ableto provide clinical supervision and support to the nursing team, advice and support while liaising with other practice staff.
To enable clinicians to safely travel to visit patients in their own home; they are obliged to ensure that your vehicle is in good working order and covered by business class car insurance (any additional premium will be paid by the practice upon receipt of a copy of the vehicle insurance cost)
Upon request, a percentage of the Nurse Practitioners time may be spent on persona land practice development activities including audit, research, team development and clinical governance.
Job Responsibilities:
Diagnosing and treating patients
Triage and treat patient swishing to see a health care professional, making any necessary referrals toother members of the primary health care team
Acute care providing clinical support/dealing with patients booked into the GP On Call clinics
Domiciliary Care seeing patients in their own homes who are too ill toattend surgery and/or performing a ward round/virtual ward round at our registered Nursing Homes
Patients with problems needing referral to secondary care should be discussed with the Usual GP before making such referral.
Ensure clinical Practice is safeand effective and remains within boundaries of competence, and to acknowledged limitations.
Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary
