Job Summary

We are looking for caring, compassionate, energetic and motivated individuals who are passionate about improving the mental health of our service users. This is a band 6 post open to Mental Health Nurses and Allied Health professionals who are currently registered with the NMC, HCPC or SWE. This pivotal role is part of the acute services pathway, focusing on evidence‑based interventions to prevent and reduce difficulties experienced by service users and carers during mental health crisis.

duties
  • Provide rapid assessment and brief evidence‑based clinical interventions.
  • Responsibility for the development, planning and implementation of brief care and treatment interventions for individual patients within the Intensive Home Based Treatment Pathway.
  • Ensure that individual episodes of care are delivered in a timely, effective and integrated manner.
  • Promote a recovery model that empowers patients, carers and relatives to be at the forefront of decision making and ownership of their care packages.
  • Undertake clinical risk assessments, based on the latest empirical evidence and compliant with local policies and procedures.
  • Act as the patient and relative advocate.
  • Enable patients and carers to manage disability, loss and change.
  • Champion patients' rights, including dignity, equality, diversity, choice and respect.
  • Ensure good documentation is provided and entered within the appropriate systems of record sharing.
Responsibilities

In addition to the main duties, the post holder will provide clinical leadership to junior team members, prioritise workloads, and utilise resources appropriately to meet service user needs. The role will involve working within a 24/7 All‑Age Crisis telephone service, Crisis Assessment Service and Intensive Home Treatment pathway.

Qualifications – Essential
  • Current professional registration with an approved professional body (NMC, HCPC or SWE).
  • Evidence of Continuing Professional Development.
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education.
  • 18 months post‑qualifying experience, including sign‑off of a preceptorship or competency framework.
  • Clinical Supervisor and recognised sign‑off mentor or clinical educator.
  • Key literacy, numeracy, and ITQ level 2 (or equivalent).
Qualifications – Desirable
  • Leadership or management qualification.
  • Post‑graduate qualification in a relevant area.
Experience & Skills
  • Significant experience working with people with mental illness in a community setting.
  • Collaborative working with service users, families and carers.
  • Providing clinical supervision and mentoring to individuals or groups.
  • Working in a multidisciplinary team and undertaking lead professional responsibilities.
  • Experience in quality improvement activities.
