Job overview:
The post is to work within and as part of the Community Health Care team supporting and working with both the clinical team and the administrative team to promote and facilitate effective leadership for the community healthcare team and supporting the discharge coordinator to provide safe and effective care and rehabilitation for patients in their own homes.
The post holder will be responsible for supporting the day to day leadership and management of the community health care team under the direct management of the discharge coordinator. The post holder is responsible for supervising and coordinating the Short Term Care staff and ensuring patient care is carried out according to the care plan and as delegated by the registered nurse.
The post holder will be responsible for assisting in the induction of new staff, conducting supervision of junior staff members, assessing competencies of staff within the team, completing rotas as required and managing referrals into and out of the service using specific criteria. The post holder will carry out delegated work in accordance with the competencies detailed within the NQF at Level 5 foundation degree level.
Direct patient care will be completed under the indirect supervision of a Registered Nurse according to the individual patient care plan, including discussing and reporting any changes in the patient's condition to the registered nurse in a timely manner. Working as part of a specialty team, the post holder will offer support to ensure patients are afforded a professional and caring service within the necessary clinical timescales.
