Support Coordinator

Pay 13.21 per hour (25838 per annum pro rata)

Permanent Part time (22.5 hpw)

Yorkshire Scunthorpe and Hull area

Typical day as a Support Coordinator

Create and develop support plans with customers and coordinate the support worker team.

Support individuals with complex needs and vulnerabilities. From homelessness, substance misuse, complex mental health and social isolation.

Assist with daily living activities; cooking, shopping, budgeting, tenancy upkeep and moving to independence.

Carrying out risk assessments, set goals and complete regular reviews and setting up tenancies for our accommodations.

Provide person-centred compassionate support without rushing.

Collaborate closely with colleagues for consistent quality support.

You bring

Passion for helping customers live their best lives with a collaborative and detail-focused approach.

Experience creating person-centred support plans and coordinating referrals.

Ability to work independently, stay calm under pressure and show resilience.

A vehicle insured for business use (mileage paid).

Our team

You'll join Laura on the Central Resource Team who has been with Home Group since 2022 and progressed from Support Coordinator through our training opportunities. She's supported by Meghan, our Client Service Manager for Yorkshire who also started as a Support Coordinator and is always available to help.

Job Details
  • 22.5 hours a week on a rota (with potential to increase to full time depending on demand) including evenings and weekends.
  • Travel and work across Yorkshire including Hull and Scunthorpe.
  • Occasionally our customers' needs mean your start and finish times may change. Where we ask you to be flexible we will always try to reciprocate.
  • Some lone working plus regular team meetings and training.
  • Confident using technology for support plans, learning and collaboration.
  • Enhanced DBS required (we cover the cost).
