We are a dynamic, friendly and specialist team and can offer you the time and opportunity to make a real difference to the quality of care that the people we support receive in community nursing home settings.

We are looking for an innovative nurse case manager to join the team, working in close collaboration with colleagues from social care, Acute trusts, primary care, care home providers and our clinical commissioning group (CCG) to really make a difference for service users and their families.

The successful candidate will be a key member of the multi-disciplinary team, responsible for providing person-centred, holistic assessment to facilitate discharge.

Our team provides therapy and nursing input across 3 different care homes throughout the city. You will be expected to move between units as required to meet service need.

Our caseload is referred into us from acute trust wards and often have complex medical history including fractures, frailty, respiratory, cardiac conditions, amputees and neurology. Our patients are with us for a short term rehab program and needs assessment to facilitate returning home as quickly as possible, facilitating discharge from hospital, ensuring their future needs are met.

There is a possibility for work hours to be increased in the future.

Main duties of the job
  • To manage a caseload of Intermediate Care Unit based patients with complex needs, using specialist skills and evidence based/patient centred principles to assess, plan, implement and evaluate interventions.
  • To provide supervision of students on practice placement.
  • To participate in the planning, delivery and evaluation of Intermediate Care services within a designated area/team, holding responsibility for defined projects.
  • To be an active member of the inter-disciplinary Intermediate Care team in the provision of patient-centred care.
  • To develop and provide clinical expertise in a designated specialism and participate in the dissemination of evidence based practice across the service.

The team is spread across 3 care homes in the city and work very holistically in a supportive team environment to maximise the outcomes for our patients. We work closely with Therapists as well as staff from private sector care homes.

Person Specification Qualifications
  • Professional Nursing Qualification
  • NMC Registration
  • Commitment to and documented evidence of Continuous Professional Development
  • To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB

Experience
  • Experience working in primary care or experience of working with, managing and coordinating care of vulnerable adults
  • Experience of working closely with other agencies
  • Experience of carrying out complex health and social care assessments
  • Experience of working with a multidisciplinary team and orchestrating care
  • Experience of working with the Single Assessment Process
  • Working with older people in the above settings.
  • Experience of working in the NHS
  • Community and Intermediate Care working experience.
  • Management of patients with complex health conditions
  • Knowledge of working with Systm One
Other Factors
  • The ability to understand and respect different points of view and work effectively with people from a variety of cultures
  • Due to the requirement to undertake home visits and transport non-portable equipment, the post holder is required to be either a car driver with a valid driving licence, or have access to an alternative means of transport
