We are seeking an autonomous, innovative, Community Mental Health Nurse to work within the North Community Mental Health Team (North CMHT) in Bradford. The team delivers recovery‑focused care under the Care Closer To Home initiative. The ideal candidate will work with Doctors, Nurses, Psychologists, Social Workers, Occupational Therapists, support and peer workers and will bring a dynamic approach to the multidisciplinary team.

The post holder will work directly with service users with severe and enduring mental health needs, supporting them in their recovery and acting as Care Coordinator.
Excellent communication skills, enthusiasm for service development and a commitment to recovery are essential.

Main duties of the job
  • Deliver community mental health care to a caseload of clients in line with Care Trust objectives and team policies with attendance and attendance.
  • Implement all Trust policies and procedures as required.
  • Receive and accept referrals, initiate, lead and coordinate assessment, and refer to other professionals when necessary.
  • Manage a personal caseload, including the administration of prescribed depot treatments (in Clinics & home).
  • Act as Care Coordinator responsible for assessment, planning, intervention and evaluation of care for a designated client group. Conduct accurate, timely assessments and ongoing reassessment of clients’ condition, considering all social, physical and environmental factors.

Person Specification
  • Registered Mental Health Nurse, Occupational Therapist or Social Worker.
  • Evidence პერიო post‑registration training in mental health needs/challenging behaviour.
  • Diploma in Nursing.
  • FLiCP/SLiP or equivalent.
Experience
  • Experience working or placed inacles a community mental health setting.
  • Experience in relevant area of mental health nursing.
  • Experience as clinical adviser to junior staff or students.
