Senior​/Staff Nurse; Homecare

Roles and Responsibilities:

• Patient Care Support: Assist the nursing team in delivering personalized and cost-effective care. Monitor and report patient progress to senior nurses.
• Patient Discharge & Follow-Up: Help with discharge planning and post-discharge care coordination. Follow up through phone reviews, home visits, and ensure community services are in place.
• Education & Self-Care: Educate patients and families on health conditions and self-care management. Provide guidance on holistic care as needed.
• Nursing Assessments & Services: Conduct nursing assessments, create care plans, and provide necessary services (e.g., wound care, medication management, dressing changes).
• Home Visit Safety & Documentation: Ensure safety and infection control during home visits. Document all assessments and services provided.
