We are offering an initial temporary contract (3-6 months) with potential permanent extension for a Right Care Navigator at Slough Council (
Location:
SL1 2EJ
).
Role summary:
The service will work with internal and external partners to identify, respond to, and maintain an accurate overview of rough sleepers.
- Provide advice and referrals to services meeting the customer’s individual needs, e.g. health care, substance misuse services, housing and benefits.
- Enable people sleeping rough to address their needs, improve health and find appropriate solutions to end homelessness, encouraging a settled life and supporting those at risk of rough sleeping.
- Be responsible and accountable for effective delivery of high‑quality outreach services for rough sleepers and those under duty in temporary accommodation.
- Undertake an assessment of needs and risk to ensure client’s individual needs are identified and met.
- Respond to immediate needs and risks by engaging clients with relevant health and support services.
- Enable clients to relocate or return to their home/area, involving specialist agencies where necessary.
- Promote rights, choice, dignity and independence, always working within appropriate professional boundaries.
