We are looking for a committed and compassionate Rough Sleeper Navigator to join our Rough Sleeping Reduction Service. This role plays a vital part in a multi-agency approach to tackling street homelessness, working closely with the local authority, police, health services and housing providers.
You will work directly with individuals who are sleeping rough or at risk of rough sleeping, many of whom have complex and multiple needs. Your focus will be on preventing repeat rough sleeping, supporting people in temporary or supported accommodation, and helping individuals move towards a more settled and sustainable way of life.
This is a front-line, community-based role involving daily travel across Slough (and occasionally outside the borough), meeting clients where they are – on the streets, in temporary accommodation, day centres and other community settings.
Key Responsibilities
Manage a caseload of people who are rough sleeping or at risk of rough sleeping
Carry out needs and risk assessments and develop person-centred support plans
Support clients to access housing, health care, mental health services, substance misuse support, benefits, employment and training opportunities
Work closely with the Outreach Team, Housing Demand Team and Temporary Accommodation Team to identify and support individuals at risk of homelessness
Visit clients in temporary accommodation, hostels, supported housing or on the streets to provide early intervention and prevent homelessness
Participate in early morning and late-night outreach shifts, including joint outreach with partner agencies and rough sleeper counts
Provide cover for the Outreach Team when required
Build and maintain strong partnerships with internal and external agencies to maximise positive outcomes for clients
Promote clients’ rights, dignity, choice and independence while maintaining professional boundaries
Maintain accurate and up-to-date case records and complete required reports
Work in line with lone working and health & safety procedures at all times
About You
You will be passionate about supporting people experiencing homelessness and comfortable working in a fast-paced, sometimes challenging environment.
You will need:
Experience working with vulnerable adults, homelessness, housing or outreach services
A strong understanding of complex needs including mental health, substance misuse and social exclusion
Excellent communication and relationship-building skills
The ability to work independently and as part of a multi-disciplinary team
Confidence working in community and street-based settings
A full UK driving licence and access to your own vehicle
Flexibility to work early mornings, evenings and occasional late-night outreach
