The Transitions of Care RN at Elevance Health is responsible for delivering patient education and disease management interventions in a virtual setting. This role is ideal for individuals who are passionate about health coaching and improving patient outcomes.

  Full-time, Monday – Sunday with mandatory 2 weekends a month (4, 10 hour shifts)

What You'll Do

• Conduct behavioral or clinical assessments to identify individual member needs.
• Identify and coordinate specific health coaching plans based on assessments.
• Interface with providers and health professionals to coordinate health coaching plans.
• Implement and coordinate coaching and care plans by educating members.
• Facilitate referrals to health professionals for behavioral health needs.
• Use motivational interviewing to promote health behavior change.
• Monitor and evaluate the effectiveness of interventions and health coaching plans.
• Direct members to facilities, community agencies, and appropriate providers.
• Refer members to catastrophic case management.

Requirements
Education:

• AS in nursing required, BS in nursing preferred

Experience:

• Minimum of 2 years in condition-specific clinical or home health/discharge planning experience

Licenses &

Certifications:

• Current unrestricted RN license in applicable state(s) required
• RN compact license preferred
