You will deliver high‑quality, person‑centred, and strengths‑based social work that promotes independence, wellbeing, and choice. Your work will align with the Care Act 2014, Mental Capacity Act, and Somerset Council’s policies, contributing to making a positive impact within adult social care.

Complete proportionate Care Act assessments, mental capacity assessments, support plans, and reviews.

Identify strengths, risks, and outcomes using a person‑centred approach.

Apply Making Safeguarding Personal in all Section 42 safeguarding enquiries.

Manage a varied caseload within statutory time frames.

Develop, implement, and review personalised support plans.

Work collaboratively with adults, carers, families, and professionals.

Lead safeguarding enquiries and undertake robust risk assessments.

Coordinate multi‑agency safeguarding responses when required.

Ensure adherence to local safeguarding procedures and legal frameworks.

Build strong relationships with health, housing, community, and voluntary sector partners.

Contribute to MDT meetings, CHC processes, and hospital discharge planning.

Complete carers’ assessments and offer support and signposting.

Maintain accurate and timely records in accordance with Council systems.

Ensure compliance with legislation, policy, and SWE professional standards.

Engage in reflective supervision, CPD activities, and practice development.

Qualified Social Worker (BA, MA, DipSW or equivalent).

Social Work England registration.

Strong working knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures.

Confident in assessment, risk management, and autonomous decision‑making.

Excellent communication, partnership working, and report‑writing skills.

Experience in adults’ statutory services (community, hospital, safeguarding, complex care, LD, MH).

Best Interests Assessor qualification or willingness to train.

Experience in strengths‑based practice and carer support.
