Medicines Optimisation Assistant – Symphony North Healthcare, Somerset

Job Summary: To support the clinical teams to improve the quality of prescribing for patients and the handling of medicines within our practices. You will follow strict protocols and apply your knowledge of pharmaceuticals to make simple changes to patients’ medication records, and to improve medication processes within GP practices. Working closely with both the prescription clerk and medicines management technician teams, you will help patients get the best from their medicines by supporting improvements to streamline repeat prescribing processes, minimise clinical risk and reduce waste.
