Location: Chilton

Siam Surgery has an opportunity for two care coordinators to join our team on a part‑time or full‑time basis, for a minimum of 22.5 hours per week. The post‑holder will provide extra time, capacity and expertise to our practice team to help us improve the care provided to our patients. The main duties of the role include:

Responsibilities
  • Working with GPs and other professionals within the practice to identify and manage a caseload of patients, particularly vulnerable patient groups with long‑term conditions that may require extra support.
  • Directing patients to locally available support and services.
  • Supporting the coordination of all key activity, including access to services, advice and information, and ensuring that health and care planning is timely, efficient, and patient‑centred.
  • Helping patients prepare for, or follow up after, clinical conversations and hospital discharges.
  • Supporting digital initiatives.
  • Promoting health education.
  • Using clinical systems on a daily basis with confidence to run searches and keep care records up‑to‑date.
Qualifications
  • Experience of working within healthcare or the community sector.
  • Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential).
  • Familiar with local resources and services, including how to access them.
  • Motivated by helping people, with care and empathy.
  • Strong listening and communication skills.
  • Strong IT skills.
  • Good attention to detail.
  • Able to work well within a team.
  • Strong organisational skills and good time management.
  • Able to prioritise workloads.

Siam Surgery is a GP practice and part of the Sudbury Primary Care Network. Located within Sudbury Community Health Centre, we provide a high level of care to over 13,000 patients in Sudbury, Suffolk and surrounding villages. We were rated good in all areas by the CQC in October 2025 and we have a high level of patient satisfaction. We are a well‑established, friendly and supportive team who take pride in the work we do.
