Job Title: Senior Social Worker – Hospital Team

Location: Stafford, UK

Salary: £31.97 per hour

Work Type: Full Time (08:00–17:00, Monday–Friday)

Contract Type: Ongoing (initial 4-month term, with potential extension)

Take your social work expertise to the next level in Stafford as a Senior Social Worker within the Hospital Team. This essential Level 2 Band 6 role focuses on supporting the West D2A beds Discharge teams, ensuring timely hospital discharges and preventing delayed transfers of care. You will be based across two private care homes in Cannock and Stafford, with opportunities to support additional locations across Cannock, Seisdon, and Stafford.

Purpose of the Role

To facilitate timely discharges from hospital settings, prevent delayed transfers of care, and provide high-quality social work interventions that promote patient independence and wellbeing.

What You Will Do

Facilitate smooth and timely hospital discharges, supporting patients’ transitions to care homes or the community.

Collaborate with Multi-Disciplinary Teams (MDTs) including nurses, therapists, social workers, and the MPFT Home First Team.

Conduct statutory social care assessments, support plans, Mental Capacity Assessments, and Best Interest Meetings.

Manage s.42 enquiries, working to enhance patient independence and prevent readmissions.

Build and maintain strong partnerships with hospital and community-based teams.

Manage your workload efficiently, demonstrating autonomy, strong decision-making, and prioritisation skills.
