CSH Surrey are looking for a forward thinking Senior District Nurse to join their Community team and play an instrumental role in the delivery of high quality, seamless, safe and clinical effective care across a defined neighbourhood within the Spelthorne area.

In this role you will be expected to provide the day-to-day management of a team of Registered Nurses, Associate Practitioner and Healthcare Assistants. You will both coordinate the team activity and also use your own clinical skills to undertake holistic assessments of patients who have complex health and social care needs.

You will ensure that the care delivery meets national and local policy and guidelines, Statutory regulations (CQC/Monitor) and professional body (NMC) requirements.

Main duties of the job

The main duties of the role include:

Effective communication across a wide variety of stakeholders, including GPs, Adult Social Care, acute and community hospitals, borough councils and the local voluntary sector.

Attention to detail, ensuring that all patient needs are met and accurate record keeping reflects this.

The triaging of patient need and the ability to prioritise clinical activity. The provision of advice and guidance to junior staff and positive role modelling within the team.

Knowledge of palliative care within the community and the importance of timely patient interventions at this time.

The recognition of when issues need to be flagged, including the requirement to work closely with Safeguarding if concerns are identified and raised.

Being open and receptive to service development, including the implementation of new ways of working and general problem solving.

Ensuring that the quality of service provision is monitored and can be evidenced. Promote the effective and efficient use of resources.

Provide the first line management for staff within the team, actively participating in HR issues as appropriate.
