The Steps to Health service in Sunderland provide an innovative and dynamic approach to individuals who are at risk or who have developed a long‑term condition and would benefit from exercise and rehabilitation to keep them well. Education complements a programme of rehabilitation to provide knowledge and support enabling patients to make healthy choices and self‑care.
We would like to invite applications for a Registered Nurse to join our team where you will play a key role working with specialists supporting assessments of patients referred to the service and initiating a plan of care.
You will be directly involved in education and in the delivery of exercise classes which can be face to face or virtual.
You will be / have:
- Registered Nurse.
- 1st Degree in Nursing / working towards.
- Experience of working with patient/clients with a Long‑Term Condition.
- Knowledge and skills in using motivational interviewing and behaviour changes techniques.
- Excellent interpersonal and communication skills.
- Experience of /able to demonstrate ability to educate patients.
- Participate in delivering training and education programmes to patient/clients specific to their diagnosis.
- Support patient/clients to develop self‑care strategies and signpost to relevant agencies as required.
- Support the specialist nursing/therapy team by completing identified intervention and review to enable effective clinical management of a defined Long‑Term Condition.
- Caseload management and participation in clinical audit.
- Multi‑disciplinary team working.

Job responsibilitiesPLEASE REFER TO THE ATTACHED JOB DESCRIPTION FOR FULL DUTIES OF THE ROLE.
- To be responsible and accountable for the delivery of safe, effective and compassionate nursing care for patients designated to their care.
- Undertake comprehensive holistic assessment of patients in both planned and unplanned situations, be able to prioritise clinical need, initiating investigations, determine a differential diagnosis and implement a plan of care reflecting evidence based guidance.
- To always work to the NMC Code.
- To supervise non‑registered staff as stipulated by the NMC under the requirements of delegation.
- Provide effective leadership and assist with the management and organisation of the team in the absence of a senior nurse.
- To comply with the trust’s vision, values and behavioural compact.
- Registered Nurse / current NMC Registration.
- Degree in nursing or working towards a degree.
