Position: Social Worker (closing date: 26/01/26)

£36,363 - £39,862 per year (Grade
8) / £40,777 - £45,091 per year (Grade
9). Grade 8 is for newly qualified. Qualified Social Worker to work in the Community Social Work Team covering the West Area of Swansea. Full time permanent post.

We are the Community Social Work Team, and we work with adults with physical disabilities, frailties or vulnerabilities who are eligible for support at home or in residential care over a longer period. The Team also supports individuals' transition from Child and Family Services to Adulthood.

We are recruiting for a qualified registered Social Worker to join our Community Social Work team, our goal is to empower and promote independence to individuals, building on their strengths and developing relationship based social work practice and creating meaningful personal outcomes.

About the role

Our team use the strength based Collaborative Communication Framework to help people and their families identify what matters to them, giving them their voice, choice, and control.

As part of this process, your role would be to complete assessments, care and support plans, reviews and contingency plans whilst following the principles of the Social Services and Wellbeing Act.

We are looking for candidates who are motivated, enthusiastic, and committed to social services. You will be passionate about relationship based social services and providing the best possible outcomes for individuals in our communities
