We are searching for a Social Worker to join our Harbour stone Enhanced Care team based in Sydney, Nova Scotia
.

Reporting to the General Manager, Social Workers focus on the relationships between people and their environments, which are integral to psychosocial, emotional, and spiritual wellbeing. Social Workers build upon the strengths of patients/families by focusing on existing coping strategies and problem‑solving skills, past successes, and personal resources.

About The Opportunity
  • Screens and assesses resident applications for admission
  • Supports the admission, transfer (to other units), and discharge of residents in coordination with the interdisciplinary team
  • Provides counseling and therapy to residents and families as appropriate
  • Develops support groups as necessary
  • Participates as a member of the interdisciplinary team
  • Prepares funding requests and reports relating to resident needs
  • Documents per policy and maintains appropriate health care and other records
  • Facilitates resident care conferences
  • Facilitates team meetings and family meetings
  • Responds to inquiries from family members and general public on admission procedures
  • Develops and maintains relationships with Department of Health, community institutions and community groups
  • Additional related duties as required
About You

In addition to placing high value on continuous improvement, collaboration and accountability, you bring:

  • Bachelor of Social Work
  • Current registration with the Provincial Association of Social Workers
  • Minimum of two-years clinical experience
  • Knowledge of social work practices, techniques, and ethics
  • Knowledge of counseling, mediation techniques and theories
  • Previous experience supporting admission and discharge of vulnerable populations with complex social situations considered an asset
