Complex Care Solutions (CCS) seeks a Nurse Practitioner for per diem in-home assessments across Collier County, FL. You will conduct comprehensive reviews, capture patient needs, and help close gaps in care to improve clinical outcomes.
Responsibilities include documenting encounters, following CMS/HEDIS guidelines, and coordinating with PCPs and health plans. Eligible employees participate in a 401K plan with automatic enrollment at 3%.
