Children's Social Work, Personal Advisor, Leaving Care Team, West Midlands Pay rate to £22.25 per hour Contract role - Leaving Care Team Pertemps are recruiting for an experienced Personal Advisor within Children's Social Work in the Telford area. This is to work in a Leaving Care Team, and you will need to have experience in a similar role, including the following job responsibilities:

• Undertake the statutory role of Personal Adviser role for individual Care experienced young people from age 16 up to the age 21 or 25.

• To be the key professional and case manager to Former Relevant care experienced young people and when needed to those young people who are Relevant or Qualifying for advice and assistance

• To work with young people leaving care and their support network to support the development of independent living skills and ensure a smooth transition to independence

• To assist and support young people to prepare for leaving care and living independently; providing support, advice and guidance to them up to age 25.

• You will need to be a driver with access to your own transport
