The Care Coordinator / Coordonnateur de l’admission role provides high‑quality customer service and manages referrals efficiently. This position is remote and the hourly rate ranges from $20 to $24.10 per hour.
Key Responsibilities Effective Referral Coordination and Data Entry – 60%- Review funder case documentation for completeness, identify missing information, and request it.
- Screen referrals for potential conflicts of interest.
- Accurately schedule assessments and treatments, confirm clients in the scheduling system, and meet SLAs.
- Upload referral and medical information, complete client profiles in Clinic Master, and ensure accurate data entry.
- Follow up with referral sources for any missing information.
- Act with integrity and align with best practices, privacy, and CBI policies.
- Collaborate with the Team Lead to resolve gaps and identify improvement opportunities.
- Communicate with clients and funders promptly about scheduling and information management.
- Use effective customer‑service strategies to answer phones/emails, take messages, and provide timely follow‑up.
- Maintain knowledge of privacy, confidentiality, and code‑of‑conduct policies.
- Build relationships with referral sources to become a trusted advisor.
- Facilitate navigation of service offerings with referral sources and clinical teams.
- Meet or exceed funder referral requirements regarding process management and operational efficiency.
- Communicate barriers in achieving KPIs to the Team Lead.
- 2+ years of customer‑service experience.
- Post‑secondary education in medical office/business administration or equivalent.
- Track record of successful experience in client experience in a multi‑site booking environment.
- Healthcare experience is an asset.
- Strong professional communication skills in English;
French is an asset. - Customer‑experience, planning, organizing, and multitasking skills.
